Affordable Care Act (AKA Obamacare):
For the 2014 tax year, taxpayers are required to obtain proof of insurance coverage for each month of the year.
This information would normally be included on Forms 1095-A, if insurance was purchased on the Exchange, Form
1095-B, if insurance was purchased "off the Exchange," (usually from an insurance company, or from Medicare or
Medicade,) or on Form 1095-C, if insurance was provided by your employer.
Please be sure to obtain this information as you gather your tax information. Penalties, premium tax credits or a
return of exchange subsidies could result if you do not have this information.
